Note: Please complete an emergency form 2018-2019
for students and provide an updated light, gas, or water bill in parent’s name (if no
bill in parent’s name, you must
complete an affidavit and homeowner must be present to sign their portion of the
affidavit, provide 2 utility bills and parents must bring a formal letter that
has been mailed to them at that address).
